Pattern Hair Loss in Women
Estimates from the American Academy of Dermatology estimate that 30 million women are affected by hair loss in the United States alone. This is more than likely a conservative number, since women are able to camouflage their hair loss more easily.
The term "Female Pattern Hair Loss" (FPHL) is preferred over Androgenetic Alopecia because the latter term is also used to describe male pattern baldness (MPB).
The blanket term of FPHL covers many different causes of hair loss. Usually occurring after puberty or hormonal changes such as menopause, post partum or discontinuation of birth control pills, FPHL is triggered by genetics and/or hormones. The female pattern usually keeps the hairline intact and thins in the frontal and mid-scalp areas. As you can see in the diagram, Ludwig I and II are far less severe than the rare Ludwig III, which fortunately only occurs in 1 in 7 patients.
The difference of baldness patterns between men and women is due to the presence of the enzyme Aromatase in the female frontal scalp which converts male androgens into estrogen.
The term, coined by Dr. Elise Olsen, "Christmas Tree Pattern" describes the appearance of female pattern hair loss. The image shows this pattern in a female client. Dr. Olsen noticed that while it usually spares the hairline, it is accentuated near the front of the scalp.
FPHL treatment, like MPB is a two-option approach. Medicines indicated for treatment include Spironolactone and Rogaine. Caution must be used with women using Spironolactone of child-bearing age due to the risk of potential feminization of male fetus. Rogaine 5% carries a risk of facial hair growth.
Best results for FPHL are surgical treatment for the appropriate candidates. Employing the use of ultra-refined follicular unit transplantation, density can be restored in areas of the scalp to enhance hair styling options. For women with MPB recessions, these hair restoration procedures can also recreate hairlines.
